Release and Waiver of Liability

The TCSPCA has developed a program whereby residents of the Turks and Caicos Islands can have their pets spayed or neutered at a low cost. Along with this surgery, your animal will also be eligible to receive its required vaccinations at a lost cost.

 

IN CONSIDERATION of having my dog/cat sterilized, I hereby acknowledge, agree and represent the following:

1. I understand the spay/neuter and all other surgical procedures have the risk of complications, including death, to my animal. I hereby assume full responsibility for any risk of sickness, injury or death of my animal due to these procedures.

 

2. I am the owner of the above animal or I'm responsible for it and have the authority to execute this consent.

 

3. I agree to indemnify and hold harmless the TCSPCA, the attending veterinarian and their staff, and any of the officers or employees of said corporate entity from any and all liability arising out of the performance of all procedures referred above.

 

4. I, acting as owner or duly authorized agent of the owner of the animal named above, hereby authorize the participating veterinarian to perform a surgical sterilization of this animal.
